Community Learning Disability Team for people with learning disabilities

About the service

The Community Learning Disability Team (CLDT) is for people with learning disabilities and works with people and carers to promote independence. They offer advice and support to people on every aspect of living with a learning disability, including:

  • making choices, finding employment and being more independent
  • healthy eating advice, health action plans and looking after yourself
  • getting the most out of local health and social services
  • budgeting and direct payments
  • meeting other people and taking part in activities
  • learning new skills
  • short breaks for you and your carer

Learning disability services are run in people's homes, health centres, nursing homes, and residential homes throughout Barnet, Hammersmith & Fulham, Kensington & Chelsea, and Westminster.

How to access the service

This service is for people who have a learning disability, are 18 years or older or moving from children to adult services and have a GP in Westminster.

If you have a health-related concern about your learning disability, you can refer yourself or ask someone to refer you to the service. They can be your family, friend, GP, social worker, or other professional.

You can make a referral by contacting CLCH SPA by email or phone. They will help you make the referral.
