NHS Homeless Health Team at the Passage Homeless Resource Centre

About the service

Homeless Health Services is an open-access outreach service that takes primary care into non-NHS settings.

Provided on the street, in day centres, hostels, and specialist GP practices, they make the NHS available to people who otherwise have difficulty gaining access to care.

The team consists of nurses and counsellors, with additional input from the Trust’s dentistry and podiatry teams, which provide walk-in clinics Monday through Friday.

This crucial service aims to relieve suffering, provide public health screening, offer help and support with rehousing, return patients to mainstream services, and eliminate the need to use accident and emergency for primary care.

The homeless health teams cover a range of areas, including:

  • counselling services - both face-to-face and group sessions
  • physical health problems
  • advocacy, supporting housing claims and tenancy support
  • screening, advice and support

How to access the service

The homeless health counselling service is a walk-in service for homeless people or people who have experienced homelessness and require help. Appointments are not required.

The service also accepts informal referrals from all agencies.

One-to-one individual and group counselling is available as drop-in sessions or appointment-based sessions.

The team are based at a number of other day centres:

  • The Passage Homeless Resource Centre
  • West London Daycentre (Seymour Place)
  • The Connection at St Martin in the Fields
  • The Market Lane Day Centre (Broadway Centre)

The team also undertake street outreach shifts and visit local hostels.

Availability

Nursing services are open Monday to Friday between 9am and 1pm.

Podiatry is available on Thursdays between 9am and 12:30pm.
