Find out about the help available to you as a care leaver.

Health and emotional wellbeing

We will support you in staying healthy and living a healthy lifestyle. After meeting with the Looked After Children (LAC) nurse, you will receive a health passport when you turn 18.  

This will contain a summary of your:  

  • health assessments 
  • health history  
  • how you can access relevant health services  
  • your health records  

The LAC Nurse will:

  • contact you and your PA for a follow-up conversation about your health and emotional wellbeing one year after your final assessment. 
  • offer support to find local NHS services 
  • help you access adult mental health services, attend hospital and community clinic appointments, including sexual health services 
  • help you maintain a healthy lifestyle through advice and support on various topics.

Your PA can:  

  • give information on healthy living  
  • give information and financial support to pay for prescriptions  
  • give information on intimacy and healthy relations and about health drop-in centres 
  • help you register with a GP and dentist 
  • support you to move from CAMHS (Children and Adolescent Mental Health Services) to adult services  
  • give information about counselling services that are available locally.  
  • if you are a young parent, we will support you in accessing appropriate services and doing what is best for your child or children.  
  • if you want support with drug and alcohol use, your PA can provide you with advice or support in accessing services in your area.

Community Navigators and Young Adult Mental Health Social Prescribers

  • In partnership with One Westminster and the Central and North West London Mental Health Trust, the service is here to help young people in Westminster feel better and achieve their health and wellbeing goals. They will work with you to set personal goals, both big and small, and connect you with community and volunteer services that can support you.

In-house psychotherapists, family therapists and art therapists

  • Individual, group and family therapy. The service and virtual school run different groups/events using the creative arts, food and sport to support physical and emotional wellbeing. We also commission projects from Elements and Dream Arts, which provide arts and drama activities for care leavers.  
  • The service and accommodation providers in Westminster work closely with Insights, an alcohol and drug advisory service that offers advice, mentoring, and fun activities to develop trusting and supportive relationships that meet clients' needs.  

NHS Low Income Scheme

  • If you are on a low income or receiving Income Support, income-based Employment and Support, Job Seeker's Allowance, or Universal Credit, you can apply for a Health Credit Certificate (HC2 or HC3) to support free dental care, free eye care and prescriptions.
