Your pathway plan
Your pathway plan is an up-to-date assessment of your needs, views, and future goals. Your first pathway plan will be created around your 16th birthday and reviewed every six months to ensure it reflects the level of support you want and need.
Your pathway plan will also involve discussions with important people in your life. Your personal advisor will give you a revised copy of your plan every six months, and you can ask them for a copy.
Your Personal Advisor (PA)
We will start allocating you a personal advisor (PA) between 16 and 18 years of age. Depending on your circumstances and wishes, this may be your social worker.
Your personal advisor (PA) will offer you support, advice, and guidance as you become more independent. Together, you will explore your goals when you leave care and how we can help you achieve your aspirations.
Your PA has a legal duty to visit you at least every two months, but we aim for more regular contact according to your needs and wishes.
Your PA will work with you until you are at least 21, and you can continue to receive support from them until your 25th birthday. We will write to you annually to remind you. If you are in a continuous course of education before your 25th birthday, we will support you until it is complete.
