Care Act Assessment  

The Care Act Assessment is a process used to determine if you are eligible for care and support services based on your needs. It evaluates how your physical or mental health affects your ability to carry out daily tasks and live independently. If eligible, you will receive a care and support plan tailored to your needs. Universal Credit

Discretionary Housing Payment
